Benefit Choices for Today & Tomorrow
At Newport Hospital, for most benefits, employees are eligible to
participate as of the first of the month following their date of hire.
Others may require a short waiting period or require approval prior to
coverage.
The cost of benefits in most cases is shared between the employee
and Newport Hospital. The employee's share of the cost is determined by
full-time or part-time employment status, the plan chosen and the number
of dependents insured for medical, dental and/or legal coverage.
Premiums are payroll-deducted either pre- or post-tax (depending on the
type of benefit).
Benefit Choices
Medical Insurance
Choosing a medical plan for yourself and your family is a personal
decision based on your specific needs. This is why Newport Hospital
provides a choice of two medical plans. These plans offer comprehensive
health care services with varying levels of benefits. You can select the
plan that best meets the needs of you and your family.
Both medical plans provide hospitalization, outpatient services and
prescription drug benefits. The main differences between these plans are
the deductibles, co-payments, co-insurance amounts, out-of-pocket
maximums and plan costs.
Medical Insurance Coverage Options

HealthMate Coast-to-Coast (PPO) - Blue Cross/Blue Shield of
RI
The PPO plan allows you to receive care from in-network or
out-of-network providers. To receive the highest level of
benefits you must obtain services through network providers. If
you receive out-of-network care, you'll share more of the costs
through deductibles and co-insurance. The PPO does not require
that you select or have your care coordinated through a primary
care physician (PCP). Most in-network hospital and diagnostic
services are covered in full and physician visits are covered
after a $15 co-payment, or $20 co-payment for some specialists.
Each medical plan offers a different prescription drug benefit
through the major pharmacies and other independent pharmacies.

Blue Chip (HMO) - Blue Cross/Blue Shield of RI
The HMO plan provides benefits only when care is received from
providers within the HMO network. The HMO plan requires that you
select a primary care physician (PCP) who will coordinate your
health care needs. Out-of-network coverage is available in
life-threatening, emergency situations only. Most hospital and
diagnostic services are covered in full and physician visits are
covered with a $15 co-payment. Out-of-network services are
available after a $250 per person/$500 per family deductible,
and covered expenses are covered at 80 percent. Each medical
plan offers a different prescription drug benefit through the
major pharmacies and other independent pharmacies

No Coverage
If you have other medical coverage and choose no medical
coverage through Newport Hospital, you can receive taxable cash
in lieu of benefits in your paycheck throughout the year.
Coverage for You and Your Dependents
When you enroll in medical coverage, you can choose coverage based on
the number of dependents you are eligible to cover:

Appropriate dependent verification documentation must be provided prior
to dependent(s) coverage taking effect.
Prescription Drugs
When you enroll in a medical plan, you will automatically receive the
prescription drug benefits of that plan. Each medical plan offers a
different prescription drug benefit through the major pharmacies and
other independent pharmacies.
Dental Insurance
Newport Hospital offers two comprehensive dental plans that include
in-network and out-of-network benefits with coverage to accommodate your
personal and family dental needs.
Dental Insurance Coverage Options

DeltaUSA Option 1
Provides 100 percent coverage for preventive and diagnostic
services, as well as 100 percent coverage for minor and major
restorative services. Provides 50 percent coverage for
periodontics.

DeltaUSA Option 2
Provides 100 percent coverage for preventive and diagnostic
services, as well as 100 percent coverage for minor and major
restorative services. Provides 50 percent coverage for
periodontics, prosthodontics and orthodontia (for dependent
children only/$1,200 lifetime maximum).

No Coverage
If you choose no dental coverage, you can receive taxable cash
in lieu of benefits in your paycheck throughout the year.

When you enroll in dental coverage, you can choose coverage based on the

Long-Term Disability Insurance
The long-term disability plan replaces a percentage of your income if
you become disabled due to an illness or injury and you're unable to
work for more than 180 days.
Long-term disability insurance coverage is provided at no cost to you.
Coverage is 60 percent of pay to $6,000/month.
Basic Life and AD&D Insurance
If you should die or become seriously injured in an accident while
you're employed at Newport Hospital, life and accidental death and
dismemberment insurance could provide you and your family with an
important financial "safety net."
Employee Life Insurance and AD&D Insurance Coverage is provided at
no cost to you. This coverage is equal to 1 times your annual base pay.
In addition, you have the option to purchase supplemental coverage based
on your annual base pay as follows:

No Supplemental Coverage
The combination of basic and supplemental coverage maximum is 5 times
your annual base pay. For those employees who take supplemental
coverage, free will preparation services are available.
Retirement Program
Newport Hospital's retirement program has been designed as a partnership
between the hospital and you to help you plan ahead for your future.
Newport Hospital will work with you, contributing and investing funds,
to help you reach your retirement income goals. The retirement program
has three components:

Choice Core Account
Newport Hospital provides an annual core contribution of 2% to
6% of annual pay based on years of credited service. You become
eligible to participate on the first day of the month after you
complete one year of service and have reached age 18. You
receive a contribution for each calendar year in which you are
credited with at least 1,000 hours of service.

Choice Matched Savings Accounts
-
Vesting
Your own contributions to your account, plus investment
earnings, are immediately vested. Newport Hospital's core and
matching contributions are fully vested after you have completed
three years of credited service.

Supplemental Tax-Sheltered Annuity Account
This program enables you to reduce pre-tax earnings and
invest your pre-tax dollars in TIAA-CREF. You can choose from 10
investment funds. Newport Hospital does not match these
contributions. The interest on your contributions accumulates on
a tax-deferred basis.
Paid Time Off
Holidays
The hospital observes the following holidays:

To be eligible for paid holidays, you must be regularly scheduled to
work at least 20 hours per week. Part-time employees will receive
pro-rated holiday time based on authorized work hours.
Time-and-a-half will be paid to all eligible employees for hours worked
on the above-mentioned holidays, provided a minimum of four hours are
worked. For Christmas Eve and New Year's Eve, any shift beginning at 3
p.m. or later will be paid time-and-a-half, provided a minimum of four
hours are worked. If you work on Thanksgiving Day or Christmas Day, you
will receive a free holiday meal in the cafeteria.
If you are benefit-eligible and scheduled to work the holiday, you have
the option to receive the holiday benefit as additional pay based on
authorized hours, or you may take time off within 60 days after the
holiday. Failure to take time off within this time period will result in
automatic payment of the benefit.
You may be given time off to observe personal religious holidays by
substituting another holiday, taking earned time, or taking a day off
without pay. Employees are eligible for various leaves of absence for
medical, family, educational, military, bereavement, or other personal
reasons.
Earned Time
Newport Hospital recognizes the need for employees to take time off from
work for purposes of vacation, personal or family illness, or other
personal needs. Compensation is provided for these absences through a
benefit called earned time.
Earned time is flexible, allowing you to choose how your paid time off
is used. Designated holidays, such as New Year's Day, Christmas, etc.,
are not part of earned time.
Eligible employees begin to accrue earned time with their first paid
hour of employment. The amount of earned time that an employee is
eligible to accrue is dependent on job category, length of service and
the regular hours of the position. Annual earned time ranges from 20 to
30 days per year, prorated for part-time employees. Employees may take
accrued earned time after three months of employment with their
manager's approval.
Once a year, employees may choose to sell up to three weeks of earned
time, in accordance with policy guidelines.

Child Care, Elder Care and Family Support

Resource and Referral Services
The Employee Assistance Program offers resource and referral
services to employees who are in need of information or
assistance for child care and elder care.

Sick Child Care
An on-site, sick-child care service is provided in the
hospital's pediatric unit for children of employees. Children
from six weeks to 14 years of age with common, temporary
illnesses are eligible for care while you are working. The
service is open 24 hours a day, seven days a week, when beds are
available.
You must pre-register your child; an updated immunization record
is required, as well as other pertinent information. There is a
small fee for registration and a minimal charge per hour when
your child uses the service.

Adoption Assistance Benefit
This benefit is designed to offer adoption assistance,
including financial reimbursement and resource and referral
services, to benefit-eligible employees who are building
families. Eligible expenses are reimbursed up to $2,500 for
full-time employees and pro-rated for part-time employees, and
limited to one per calendar year, with lifetime maximum of two
covered adoptions.

Dependent Care Flexible Spending Accounts
You can contribute, on a pre-tax basis, up to $5,000 each
calendar year in a dependent care spending account to pay for
eligible dependent care expenses, including services for child
care and elder care. A convenient debit-card option is
available.

Dependent Life Insurance
The dependent life insurance plan pays you a benefit to help
cover expenses if your spouse or child dies. If you choose
coverage for your children, the plan covers all of your eligible
children for the same cost.

Long-Term Care Insurance
Long-term care is not one service, but many different services
aimed at helping people who have chronic conditions and are
unable to care for themselves. Services may be needed within the
home for day-to-day activities or in a nursing home if special
attention is needed. Long-term care services are not always
covered by major medical plans or are severely limited within
Medicare and Medicaid.

Education Assistance
The Education Assistance Program is designed to encourage
lifelong learning and the professional development of employees.
Newport Hospital provides reimbursement for continuing education
in the form of collegiate courses, programs, and workshops, as
well as acquisition of professional continuing education units
(CEUs). All benefit-eligible employees who have completed the
six-month introductory period prior to the first scheduled class
are eligible for education assistance. You must be actively
employed at the time the course is completed in order to receive
the benefit. Upon proof of successful completion of an approved
course, Newport Hospital will reimburse 75% of tuition costs,
including required fees, up to an annual maximum of $1,250 for
full-time employees and $625 for part-time employees.

Educational Development
Newport Hospital recognizes the need to provide education to
expand and enrich job knowledge and skills. Prior to each
academic year, the education department conducts a needs
analysis with the directors and managers of all departments to
determine learning needs. Conferences, workshops, satellite
programs, and skill-building sessions are developed to help meet
some of those needs. These programs are open for all employees
to attend. Many of these programs provide you with CEUs, and a
record of all the programs you attend is maintained by the
education department. Every twelve months, you will be provided
a hospital-wide and department-specific safety training program.

home/condo/renters all through the convenience of payroll

HIV Insurance
HIV insurance provides you a lump sum payment should you
test HIV positive while covered by the plan. The benefit
provides 24-hour coverage and is payable in full at the time of
the claim. If you elect this coverage, the insurance company
will require a confidential, valid, negative test result for
HIV. The test must be performed by a representative from the
insurance company.

Employee Fund
Whether it be a fire that has destroyed your home or a
serious illness that has stricken your child, your fellow
employees want to help. The Employee Fund was created to provide
financial assistance when an unexpected catastrophic event has
left them with nowhere else to turn. Unlike employee loan
programs, money granted through the Employee Fund does not have
to be paid back. The fund is managed by a committee of diverse
employees who determine whether to grant or deny a request based
on established guidelines.
